On Your First Visit – Evaluation
You will be asked to arrive 15 minutes early to your first visit in order to complete the paperwork. You may also print the forms below (see Patient Forms) and bring them in with you already filled out. We ask that you still arrive early because we will also need you to provide the following:

• Driver’s License / ID Card
• Insurance card
• Physician referral
• Applicable co-pay or deductible
• Patient Forms (click to the left)


Insurance
HDPRG accepts all major health insurances, workers compensation carriers, Tri-Care and Medicare. The company also contracts with Morongo Unified School District to supplement its mandate to care for their students with special needs, and with Inland Regional Center for children ages 0-3.


What to Expect

Your first visit will be an evaluation which will be performed by one of our licensed Physical Therapist. It is wise to wear or bring comfortable loose clothing that allows us to get to the affected area. The initial evaluation will take approximately one hour, or slightly longer including the paperwork. It could take longer if we are seeing you for more than one body part.
The appointment will consist of a discussion with the Therapist regarding your medical history and background leading you to us. The Therapist will do a physical exam of the affected area or condition which includes measurements of strength and range of motion. The initial visit may also include a short treatment of manual therapy or modalities for pain control and some exercises for you to start at home.
