What is an Adult Day Center?
At Sunset Shores we offer a program of Day Health Services to people unable to live independently in their own home. Key support services are provided for the elderly and the caregiver. We offer a comprehensive program with Medical Services and Social Activities for the elderly. Adult Day Centers are utilized to serve people who don’t necessarily need the continuous care of a nursing home but who may require some supervision during the day. People who can benefit from Adult Day Centers may be in need of supervision during the day or may require help with self-care for example, they may be:• Home alone and in need of socialization
• Returning home after a hospital stay
• Partially dependent on others and unable to manage personal care
• Physically or mentally impaired
Families benefit also when utilizing a Day Center for a Loved One for example, they receive:
• Relief from care giving responsibilities
• Emotional support and peace of mind
• Medical support and continual updates from the nurse at the center
• Bi-lingual support to clients and families
What does a medical Model provide?
• On-site Registered Nurse• Individualized Client Plan of Care
• Health monitoring including, assessment of vital signs & medication management
• Individual client health counseling
• Assistance with personal care needs with trained Certified Nurses aides
• Caregiver Support and referral services - Every 2nd Wednesday of the month.

Alzheimer’s Association and the Connecticut Area Agencies on Aging
