(Cost will be $50 for sick visit and $75 to $95 for well child exam)

• Evening Clinic

For children residing in Collin County who do not have health insurance and never had Medicaid or CHIP (if terminated from Medicaid/CHIP for financial reason we will accept them only if they have a denial letter).

Must bring proof of residency such as a utility bill or apartment lease. Ages 0-18 years/ Por 0-18 anos

For grant purposes, if you are a citizen of the United States or legal resident, please
bring the child’s social security card or birth certificate.
