In order to provide you with the best possible care, we need your most up-to-date information. If you are a new patient, or an established patient you will need to update your demographic, insurance and medical forms at the time of your visit. We are pleased to offer you the opportunity to print our forms directly from this web page, prior to your visit. Your most current insurance card and a form of identification must be presented along with your new forms. Please bring the forms completely filled out and signed, Click on all the links listed below, enabling you to print of the forms.

In an effort for our providers to have your complete medical history at your visit, we ask that you obtain these records from your prior physicians office. We have provided a link to download a "Medical Records Release From". Please click on the link below to print out the necessary form to obtain your records. You must complete and sign this form and send it to your previous health care provider. Keep in mind some offices take 2-4 weeks to process your request for records.
Appointments
You MUST arrive 15 minutes prior to the time of your appointment. You may be asked to reschedule your appointment if you arrive late. If your visit is not canceled within 48 hours prior to your appointment, you may be charges for the missed appointment.
We ask for a 24-hour notice for cancelled appointments. Reminder calls for patient appointments are done as a courtesy only and therefore might not be done for every appointment. We ask the new patients or referral patients bring their previous medical records with them or have them mailed before their scheduled appointment. All patients that need referrals are responsible for obtaining their own referral and need to have it before their scheduled appointment.
After Hours
We are available after hours through our answering service. Patients can call the main number (413) 499-8570 and they will be connected to our answering service. Phone calls will be returned for emergencies by the on-call physician.
Prescription Refills
Your drug prescription refill request can be called in to our Prescription Refill Phone Line 24 hours a day. To access that line simply call (413) 499-8570, option 3. Please leave your name, date of birth, pharmacy, name of your prescription, dose and phone number where we can reach you. Please allow 24 hours for your prescription to be called in to your pharmacy,
If you need a new prescription, please call (413)-499-8570 during office hours. These changes can only be done during office hours and may require an appointment.
