Patient Information

First Visit

Your initial visit will consist of a New Patient Exam with the dental hygienist and the dentist. This helps us to assess your current oral health and existing dental conditions. Please assist us in providing the following information at this visit:

• A list of all medications you are presently taking
• Insurance information - including insurance carrier, policy
  and certificate numbers
• When your last dental check-up was done and are there any
  recent x-rays that we could request from your previous office

A thorough medical history is an essential part of treatment planning. Please arrive early so that all this information can be collected. If you have any medical conditions that require you to take pre-medication, please inform us before your first visit.


Scheduling

We will try to schedule your appointments as promptly as possible. If you have an emergency situation, every attempt will be made to see you that day. To accommodate all of our patients needs, please be advised that we require 24 hours notice to change an existing appointment. Continuous no shows or short notice cancellations may result in dismissal from our practice.


Financial Policy

For your convenience, we accept assignment from most insurance carriers. This means that we electronically submit an insurance claim and receive payment directly from your insurance. Our fees are based on the previous years ODA suggested fee guide. Please be advised that your dental insurance plan is a contract between your employer and the insurance company; it is impossible for us to know exactly what dental services will be covered. Some insurance plans have a deductible or co-payment and most plans have an annual maximum which varies from policy to policy. It is important for you to be aware of all of these details regarding your coverage. Our office accepts Visa, Master Card, American Express, Interac, cash, and cheques.
